Job Description

Position Title:Home Care Support WorkerPosition Summary
Department:Health & Social CareYou will work within a set geographical area to provide an outcome based and person-centred service to enable service users to live an independent life through a range of support services, including personal care, emotional and physical support within the service users own home, working inline with regulatory frameworks and that of Faith community Healthcare own policies and procedures
  Supervisor:Registered Manager/ Deputy Manager/      Field Based Supervisor
No.Responsibilities and Accountabilities: –
1.Provide independence by enabling service users to maintain their skills, actively encouraging service users to make informed decisions about their own life.
2.Support service users with all aspects of personal care including: – Washing, bathing, or showering,Continence care,Support with dressing,Support getting in and out of bed and general mobility, Shaving,Preparation of meals and support with eating and drinking,   In addition, non-persona tasks may include. Ironing, shopping, washing upCollection of prescriptions and shopping from Faith Community Healthcare Ltd office,Support service users with social interactions and outings where necessary
3.Administering and supporting service users with medication inline with Faith Community Healthcare Ltd Medication training and policy.
4.Ensure duties undertaken are appropriate in meeting service users’ needs and outcomes as detailed in their support plan, reporting any changes in health, behaviour, or wellbeing to the office without delay.
5.Immediately report any safeguarding or poor practice concerns to the office without delay.
6.Attend and contribute to service user review meetings where appropriate.
7.Maintain accurate records including daily logs and Medication Administration Records (MAR Chart) alongside Topical Cream Charts. In line with Faith Community Healthcare Ltd Record Keeping, Data Protection and confidentiality policies in accordance with the Data Protection Act 1998 and the General Data Protection Regulations of 2018.
8.Attend and participate in all specialist and mandatory training as well as taking responsibility for your own on-going learning and development. To also attend supervisions and staff meetings.
9.To provide weekly availability promptly to the Registered Manager, to work on a flexible rota pattern based around the service user needs which includes alternate weekends.   

Faith Community Healthcare Ltd provide fully paid training including shadow shifts, Faith Community Healthcare Ltd will issue you with 2 sets of uniform and a branded fleece. Faith Community Healthcare Ltd provide an on-call service which is open 24 hours a day 7 days a week. Faith Community Healthcare Ltd operates the service between the hours of 07:00 – 22:00
